ACKNOWLEDGEMENTS

Parent/Guardian intends to enroll their child(ren) in Maryland International Day School and executes this contract for the full year, understands that enrollment in the MIDS program is a contract to be responsible for tuition for my child for the entire school year. Refunds due to withdrawals or expulsion from the program will be handled according to the School’s policy on withdrawal as stated in the Parent/Student Handbook.

Parent/Guardian has received and agrees to abide by all policies of MIDS as outlined in the Parent/Student Handbook and Parent Contract.

The Parent/Guardian grants permission to MIDS to use the image of the enrolled student(s). Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the MIDS Web site.

The Parent/Guardian understands that the enrolled student(s) will be taking field trips as part of student’s education at MIDS and hereby grants permission for student(s) to participate in all such field trips and also grants permission for student(s) to be transported by MIDS personnel, if necessary, in order to participate in such field trips.

Parent/Guardian agrees to support MIDS with my time and resources, to participate in fundraisers and to fulfill my MIDS hours requirement.

Parent/Guardian warrants, represents, and agrees that Student(s) is in good physical and mental condition, and has no disability, impairment or ailment that would prevent Student(s) from engaging in all activities related to the classes for which he/she is registered. Parent/Guardian expressly agrees that participation in all of the MIDS classes and facilities are at parent/guardian and student’s sole risk. Parent/Guardian agrees to maintain health insurance for Student(s) and will furnish information regarding said policy to School.

By filling out this agreement Parent/Guardian hereby present themselves as the authorized and legally responsible party for the Student(s). Parent/Guardian also represent that Parent/Guardian has never been convicted of, nor is under indictment for any crime involving violence, force or sex-related crimes involving a minor.

This agreement is effective from the date of acceptance until June 30, 2020.
